Policies and Compliance

StraightUp Counseling provides a secure HIPPA compliant patient portal to ensure all of your information is stored safely. We have policies in place that have been developed to serve as protection for yourself as the client and the practitioner. Please note that these policies and compliance efforts have been put into place to ensure all persons seeking treatment with us are treated fairly and provided a safe experience to manage their mental health needs. Some of the policies in place include but do not limit the following:

Cancellation Policy

Please note that clients who NO-SHOW for their appointments and fail to cancel will be charged the full rate of the scheduled service. No Exceptions. Clients will be charged and expected to pay a flat rate of $50 for any appointment that is not cancelled or rescheduled before 24 hours of service.  Clients who arrive after the time of their scheduled appointment, will still receive services but ONLY for the remaining time allotted for their session. These policies are put into place to ensure all clients of StraightUp Counseling are treated fairly and are receiving services for which they’ve signed and agreed upon.

Late Policy

The practitioner will wait no longer than 15-minutes to begin a session. In the event that you are more than 15-minutes late for your appointment time, the session will automatically be cancelled and you will be charged a flat rate of $25. No Exceptions.

Reduced Rates

Please share any struggles or barriers to service fees during the phone consultation and/or initial intake to be provided options for reduced rates and sliding scales.

Will be a document available in the patient portal to sign and agree to mental health treatment with StraightUp Counseling.

 
 

Informed Consent for Online Therapy

A document that will be accessible through the patient portal to sign and agree to remote services for this time and until in-person services become an option.

HIPPA - Rights & Responsibilities

The privacy and confidentiality document that will be available in the patient portal to explain your rights and responsibilities as a patient. This document will also be signed to share your understanding of the practitioners obligation to keep your information both verbal and written private and confidential.
